The short answer: catch one of the evening shows of the Astronomical Clock, walk to Charles Bridge, then eat and listen to live music. Everything below is a short walk from Malé náměstí, the little square next to Old Town Square.
An evening walk
Old Town Square and the Astronomical Clock. Every hour from 9am to 11pm, the clock strikes and the procession of the apostles appears in its two windows. The 11pm show is the last one of the day. Look up at the twin towers of the Týn Church across the square.
Charles Bridge. From Malé náměstí, follow Karlova Street to the river, about a 7-minute walk. The bridge is open to walkers day and night, with Prague Castle across the river.
